A 40mm watch with 50mm lug-to-lug overhangs a 6.5" wrist. A 41mm watch with 46mm lug-to-lug fits cleanly. The right number to look for is lug-to-lug distance, not case diameter alone. Our cutoff is L2L ≤ wrist circumference × 0.95.
We've also weighted thinness: a 39mm 9mm-thin watch fits and looks proportional, while the same 39mm at 14mm thick reads top-heavy. Heritage reissues (which were originally designed for smaller wrists) dominate the list.

Grand Seiko SBGW231 is the smaller hand-wound 9S64 reference. 37mm × 44mm L2L × 11.6mm thick. Heritage-leaning dial, 72-hour reserve.

Tudor 1926 in 36mm is the brand's small-wrist dress option. ETA-derived movement, applied indices, 100m water resistance.
Smaller Max Bill, properly proportioned.
Junghans Max Bill Hand-Winding at 34mm × 41mm L2L × 8mm thin. The smallest Bauhaus reissue available at the price.
38mm hand-wound field watch.
Hamilton Khaki Field Mechanical: 38mm × 47mm L2L × 11mm thick. The cheapest legitimate small-wrist field watch.
35.5mm Bauhaus dress watch.
Stowa Antea KS at 35.5mm is one of the few legitimately small Swiss-style dress watches in the under-€1,500 tier. Hand-wound Peseux 7001 base.
42mm case but the cushion shape wears small.
Doxa Sub 200's cushion case shape gives it 42mm width but only 44mm lug-to-lug. Wears closer to a 38mm watch with 47mm L2L. The exception that proves the rule.
